Levy, who was on bail, had also sexually assaulted a prison staff member and was released by the prison service. The Metropolitan Police, British Transport Police and Crown Prosecution Service have admitted failings in the handling of the case. The IOPC has served a gross misconduct notice on a Met Police constable and a misconduct notice on a detective sergeant. Detectives have exhausted numerous lines of inquiry and found no evidence linking Levy to any further unsolved incidents.
